Improvements in Quality Care

MCRG’s Strategic Plan for Quality, was developed using measurable objectives that maximize the potential for meaningful data. Goals are measured by industry standards (hospital comparison, benchmarks, ACR group results, etc.) and/or in comparison to historical MCRG data. CMS quality measures, ACR peer review and communication of critical results are among the metrics audited to ensure that patient reports are appropriate, accurate and timely. From 2018 to 2020, MCRG physicians performed over 84,000 peer review cases, with variance rates well below the national average. MCRG’s Quality Committee, steered by a physician Quality Officer, meets quarterly to discuss the progress of quality initiatives and address quality issues (peer review, standardized templates, procedure guidelines, safety issues, surveys, etc.).

EXAM STANDARDIZATION: The Quality Committee is actively developing a protocol to standardize the acquisition and reconstruction of brain CT’s. This project is intended to reduce the likelihood of overlooking subtle abnormalities by increasing their conspicuity, for example isodense/small subdural bleeds and linear skull fractures. A second benefit will be an overall reduction in radiation dose per patient.

ENTERAL CONTRAST: In collaboration with the South Seminole Hospital’s Quality Office, a process has been developed to improve the safety of enteral contrast usage in radiology. A pilot program was launched to test the effectiveness and feasibility of a screening tool used on every patient. The data confirmed its effectiveness in flagging patients at increased risk of aspiration. It has recently been introduced at Dr. P. Phillips Hospital and will be introduced throughout the entire MCRG practice within the next six months.

CESSATION OF GASTROGRAFFIN USAGE: The harm associated with Gastrograffin aspiration is well documented. To decrease this risk we implemented a practice of only using Gastrograffin per rectum except in rare special circumstances such as therapeutic small bowel series to relieve obstruction.

ASPIRATION PNEUMONIA TASK FORCE: The Orlando Health system is vigorously evaluating new measures related to aspiration pneumonia. MCRG’s Quality Office was key in developing the first component of that effort – the production of a system-wide alert to all team members based largely on the Enteral Contrast Screening Tool we developed. We are looking to the next phase of our work to institute measures throughout the system.

G-TUBE PLACEMENT AND CARE IMPROVEMENT: To evaluate a cluster of cases related to G-tube positioning, a collaborative team was created consisting of Orlando Regional Medical Center quality officers and representatives from MCRG. After reviewing specifics of the events, the team discovered several opportunities for improvement and have changed several significant aspects related to placement, care and follow up of G-tubes.

EVALUATION OF OBSTRUCTIVE UROPATHY PATIENTS: The MCRG Quality Division has partnered with the ORMC Emergency Medicine and Urology departments to create a clinical algorithm for patients presenting to the emergency department with symptoms of urinary obstruction. The main goal was to drastically decrease the radiation dosage from repeated abdominal CT scans. This lead to the development and implementation of a low dose CT protocol, which included the development of a standardized reporting template. The dosage of these scans is 5-10 times less than a conventional scan. As these patients can undergo multiple episodes of symptoms, the reduction in dosage will greatly decrease their risk of harm associated with radiation exposure.

Through affiliation with SR, MCRG adds value for its practice and hospital partners in many ways, including participation in the SR Patient Safety Organization (PSO), the Group Purchasing Organization (GPO), SR Sub-specialty Teleradiology Partners (SRSTP) collaborative national reading pool, and SR forums in which MCRG radiologists can interact with their peers on both business operations and quality discussions. These products have been proven to bring value back to existing SR member groups that ultimately benefit the patients, physicians, and hospitals that put their trust in them.
